Initial Comments:
An offsite follow up survey completed on 3/1/24, found that The Exurbia Group had corrected the deficiencies cited under PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart H, Home Care Agencies and Home Care Registries. The deficiencies were cited as a result of a complaint investigation completed on 10/19/23 and a follow up survey completed on 12/19/23.
